Mental health leave: seeking care and managing work evidence
A practical guide to discussing mental health, work capacity, privacy,
By Doccio Clinical Team · Sources reviewed October 2026
A mental health problem can make you unable to work and may support sick leave. A doctor assesses symptoms, functioning and safety and decides whether a certificate is appropriate. Check workplace evidence requirements while planning care and follow-up. A document addresses the supported absence; it does not replace ongoing mental health support.
A certificate can document a supported period of incapacity. It is also worth using the conversation to understand what care or follow-up would help, especially if the difficulty is recurring or affecting more than work.
Describe what has changed for you
Begin with your own experience: what feels different, how long it has been happening and what you are finding difficult. Explain the effect on concentration, sleep, work tasks or everyday activities where relevant. You can bring brief notes if it is hard to organise your thoughts during a call.
Do not select symptoms from an online checklist to secure a document. Accurate information gives the clinician a better basis for assessing your needs. If you have already received care, tell the doctor about the existing plan, relevant records and any recent changes. Remote assessment still needs to meet professional standards. Explain the work you actually do
A job title may not describe the demands that matter. Explain whether your work involves driving, operating equipment, making safety-sensitive decisions, public contact or sustained concentration. Tell the clinician about any immediate concern affecting your safety or someone else's.
The doctor considers the clinical circumstances and what can reasonably be said about work capacity. They may recommend time away, further assessment or another care pathway. A requested number of days is not automatically the appropriate duration, and a certificate should be supported by the assessment. Avant: Medical certificates — your responsibilities
A mental health day is not a separate legal entitlement
The everyday phrase can mean different things. Under the usual paid sick-leave framework, the relevant issue is inability to work because of personal illness or injury. Having a difficult day, wanting a break and experiencing illness are not interchangeable descriptions of every situation. Fair Work guidance on paid personal/carer’s leave
You do not need to settle the legal classification before seeking care. Describe the facts to the clinician and check employment entitlements with payroll or an adviser. A paid balance, an eligible reason for absence and any evidence requirement are separate matters. The certificate does not create paid hours that are not available.
Discuss privacy before the document is finalised
Tell the clinician who will receive the certificate and whether a specific form is required. Ordinary sick-leave evidence should address its purpose without unnecessarily sharing your full clinical history. Avant's certificate guidance says a general medical certificate should not include the diagnosis, while recognising that other kinds of applications can require more information. If the employer asks for a separate fitness-for-work assessment or detailed medical information, obtain advice about that actual request. Read whether a certificate needs a diagnosis. Privacy protections matter, but a general article cannot settle every employment or workplace-safety situation. Legal Aid NSW: Employment problems: requests for medical information or examinations
Tell work promptly without waiting for paperwork
Fair Work requires notice as soon as practicable, with the expected duration of leave. Evidence can be requested for one day or less. If you do not yet know how long you will be away, explain that you are seeking assessment and will update the employer when you have clearer information. Fair Work guidance on notice and medical evidence
Keep the communication focused on the absence and relevant arrangements. If you need to hand over a task, discuss a practical way to do that without implying you remain available throughout sick leave. Working from home does not remove the underlying leave framework.
What if work itself is part of the problem?
Explain the relationship you notice between work and your symptoms. The doctor can assess health and discuss care, but an absence certificate does not determine whether bullying occurred, establish an employer's legal liability or resolve a workplace dispute.
You may need both clinical support and employment advice. Keep the roles clear: a treating clinician addresses health, while a union, Fair Work or an employment adviser can help with workplace questions. If the request concerns a compensation claim, read the workplace injury documentation guide before assuming a standard certificate is sufficient.
Plan what happens after the certified period
Ask what follow-up is recommended and what should prompt earlier review. If symptoms continue, contact an appropriate clinician rather than assuming another certificate alone is the next step. A longer or more complex problem may need continuity with your usual GP or another treating professional. Medical Board telehealth guidelines
If returning to particular duties raises concerns, discuss them before the return date. The end of an absence period does not answer every question about restrictions, adjustments or a specific clearance assessment. Ask the employer exactly what information it needs.
Seeking help through Doccio
Choose doctor care when you need a broader discussion, and explain any certificate request at the start. Review medical certificate assessments and current fees to understand the available pathway. A doctor decides what is appropriate; no particular certificate, diagnosis or period of leave is guaranteed. Doccio’s published service pricing
If you or someone else is in immediate danger, call 000. Healthdirect lists after-hours and crisis support, including Lifeline on 13 11 14. Do not wait for a routine certificate appointment when urgent support is needed. Healthdirect’s after-hours healthcare guide
Prepare the work conversation separately
Decide what your employer needs to know now: that you are unable to work, the expected absence and when evidence will be provided. You do not need to prepare a full clinical account for a supervisor before seeking help. Ask the appropriate workplace contact about the evidence process.
Keep the clinical conversation fuller. Tell the doctor about symptoms, safety, functioning and existing care, even if you plan to share only limited information at work. The anxiety check-in guide can help organise that discussion without requiring you to choose a diagnosis in advance.
Use time away as part of a plan
Ask what support or follow-up is appropriate during the absence. If the problem is recurring, discuss who will coordinate ongoing care and how to access recommended services. A certificate may address the immediate administrative need without explaining what happens next clinically.
If symptoms worsen or you feel unsafe, seek urgent support rather than waiting for the leave period to end. Call 000 for immediate danger; Lifeline is available on 13 11 14 for crisis support. Healthdirect's anxiety and support information
Think about the return before the final day
Consider which duties, hours or work situations may need discussion when you return. Bring those questions to the treating clinician and the appropriate workplace process. Avoid assuming that a short absence will automatically resolve an ongoing workload, health or safety issue.
Keep any workplace concern distinct from the medical opinion. A doctor can assess your health and capacity but does not determine an employment dispute. If legal or workplace advice is needed, seek it through an appropriate service while continuing the health follow-up.
When explaining the problem feels difficult
Write a few sentences about what has changed and what you are struggling to do. You can read them during the appointment or ask a trusted support person to help you prepare. The aim is an accurate account, not a polished story or a list of symptoms selected to secure leave.
Discuss your next step with a doctor
Read about Doccio's online doctor consultation and book an online doctor appointment for a suitable non-emergency concern. Bring the relevant history, records and questions from this guide. A doctor decides whether advice, treatment, a document, testing or in-person care is appropriate. Check current fees and service information before booking. If symptoms are urgent, use the appropriate urgent or emergency service instead of waiting for a routine appointment.
