Gout is an inflammatory arthritis caused by urate crystals in a joint. An attack can produce sudden, intense pain, swelling and warmth, often in the big toe but also elsewhere. A hot, swollen joint needs assessment because infection and other joint problems can look similar and may require urgent care.
Is every painful big toe gout?
No. Injury, infection and other forms of arthritis can affect the same area. A doctor considers the pattern, previous episodes, examination and selected tests. A blood result alone is not always enough to establish the cause of an acute swollen joint.
Tell the clinician when the pain started, whether you have fever and whether you can put weight on the joint. Mention kidney problems, previous test results and all current treatment. A first episode or an episode that feels different from usual deserves careful assessment.

How are attacks and long-term prevention different?
Relieving an acute attack and reducing future attacks are separate parts of a care plan. A doctor decides what is appropriate for each, considering your medical history. Do not start, stop or change established treatment based only on whether the joint hurts that day.
Food and alcohol patterns can be relevant, but gout is not simply a failure of willpower or one food choice. Ask about the role of kidney function, other conditions and ongoing monitoring. Keep a record of attacks and how they affect walking, sleep and work so the plan can be reviewed meaningfully.
When is it urgent?
A hot swollen joint with fever, feeling unwell or inability to use the joint needs prompt in-person assessment. Do not wait for a routine online appointment if infection is possible. Telehealth can help review an established plan, but a physical examination or joint fluid test may be needed to determine the cause of a new attack.
