A Centrelink medical certificate is medical evidence used by Services Australia when assessing how a health condition affects a person's capacity to meet relevant requirements. The SU415 form is one example. It is different from a brief employer sick note, and submitting it does not automatically guarantee an exemption, payment or claim approval.

Why might a specific form be required?

Services Australia needs information relevant to the particular assessment, which may include diagnosis, functional impact, expected duration and capacity. A generic certificate stating that someone is unfit for work may not answer all required questions.

Check the exact form or request in your Centrelink correspondence. Bring it to the clinician and explain the deadline. Do not assume an online medical-certificate product includes completion of detailed government forms. The doctor may need records, a longer consultation or ongoing knowledge of your condition.

For the Australian guidance behind this explanation, see Services Australia: Centrelink Medical Certificate form (SU415) - Services Australia.

Who decides whether it is accepted?

The clinician provides medical information they can support. Services Australia applies the relevant administrative rules and makes the decision about requirements or benefits. A doctor's recommendation is important evidence, but it is not the same as an approval from the agency.

Ask Services Australia how to submit the document and whether your obligations continue while it is being assessed. Keep a copy and a submission receipt where available. If more information is requested, clarify the missing details rather than repeatedly submitting the same document.

Can Doccio complete it?

Check the service's current scope before booking for this purpose. This glossary does not confirm that Doccio completes Centrelink forms or guarantees an exemption. Your regular GP may be best placed to describe a complex or ongoing condition accurately.

Read Doccio's medical certificate service information and explain the specific form when enquiring. A doctor decides whether they have enough information to complete it, and Services Australia decides what the evidence means for the relevant requirement or claim.

Where to go next

Read medical certificates to understand the relevant service. A doctor decides whether a consultation, examination, referral or another pathway is appropriate for your situation.
